Understanding Uveal melanoma
Many uveal melanomas are discovered during an eye examination before the person notices a change. The choroid is the most common site, but iris and ciliary-body tumors have different clinical features. The absence of visual symptoms cannot establish that a lesion is harmless. [1] [2]
Care has several separate goals: control the tumor in the eye, preserve useful function where possible and assess risk of spread outside the eye. Successful local control and good visual acuity do not guarantee that metastatic risk is absent; neither does a risk estimate mean spread has definitely occurred. [3]
One condition. Different experiences.
Explore different selected symptom features, then read how they may vary. These examples are not stages or a clinical test.

No noticed change
A tumor may be found incidentally at a dilated examination. A clear-looking scene is therefore a legitimate experience and should not be replaced by an alarming universal cancer overlay. [1]
Blur, floaters or flashes
Some people notice reduced detail or moving spots, depending on associated changes in the eye. These symptoms overlap with common noncancerous retinal problems and require examination. [2]
A front-of-eye change
An iris lesion or change in pupil appearance may be visible externally. An external spot is anatomically different from a spot the person necessarily sees in their own visual scene. [1]

Specialist examination and imaging characterize the internal lesion.
- Local goal
Treatment aims to control the eye tumor with individualized functional tradeoffs.
- Whole-person plan
Surveillance, systemic options and practical support address different needs.
What is happening—and what remains uncertain
The uveal tract contains pigment-producing cells within the iris, ciliary body and choroid. Malignant growth in these tissues can affect surrounding eye structures or produce secondary retinal changes; symptoms depend on location and complications rather than cancer size alone. [1] [3]
Tumor features and selected molecular tests can help estimate metastatic risk. A genetic result from tumor tissue serves a different purpose from an inherited-family test or the blood HLA typing used to determine eligibility for tebentafusp. These tests should be explained separately. [2] [4]
What can look similar?
Nevus versus melanoma
Most choroidal nevi are benign. Specialists use lesion characteristics, imaging and sometimes documented growth to determine concern; a freckle is not automatically a melanoma and a home photograph cannot classify an internal lesion. [2]
Uveal versus conjunctival melanoma
The uvea is inside the eye, while the conjunctiva covers its surface. Cancers from different tissues have different assessment and treatment pathways even when both are called ocular melanoma. [1] [3]
Local treatment versus metastatic therapy
Plaque radiation, other local radiation or surgery treat the eye lesion. Drugs or liver-directed procedures for metastatic disease answer a different question and should not be presented as interchangeable alternatives for every newly diagnosed eye tumor. [3] [4] [5]
Similar-looking symptoms can have different causes. A matching picture cannot establish a diagnosis.
What an assessment may involve
- Expert examination and imagingOcular-oncology assessment may use dilation, photography, ultrasound and OCT to characterize the lesion and associated retinal findings. Diagnosis often relies on the clinical and imaging pattern rather than requiring a biopsy in every case. [1]
- Discuss biopsy and prognosisA tissue sample may provide prognostic molecular information even when not needed to establish the clinical diagnosis. Ask what the result could change and what procedural risks apply; a risk category is not a precise personal survival clock. [2] [3]
- Clarify systemic follow-upThe oncology team arranges staging and surveillance appropriate to the tumor and risk context. Ask who coordinates imaging, how results are communicated and what symptoms should prompt contact between scheduled visits. [3]
What may help—and what is still being studied
Local eye treatment is individualized
Radiation and surgery choices depend on tumor size, site, other eye findings and overall circumstances. For medium choroidal tumors in COMS, plaque brachytherapy and enucleation did not show a survival difference; that does not mean visual consequences or suitability were identical. [3]
Biomarker-selected systemic treatment
Tebentafusp is approved in the US for adults with unresectable or metastatic uveal melanoma who are HLA-A*02:01-positive. Its survival evidence does not apply automatically to people without that marker or to adjuvant treatment after every localized tumor; cytokine-release and other serious risks require supervised care. [4] [6]
Liver-directed treatment has specific limits
The US HEPZATO KIT indication concerns selected unresectable liver metastases, with strict disease-distribution and safety criteria. It is a specialized procedure with boxed warnings and a restricted safety program, not a general injection for all ocular melanoma. Country approvals and access differ. [5]
What this can mean for daily life
Keep the care map manageable
Write down the eye-oncology contact, systemic-oncology contact and surveillance plan. Ask which team handles treatment side effects, vision changes and scan scheduling so responsibilities are clear. [3] [7]
Discuss function and rehabilitation
Ask about expected visual change from the tumor and treatment, low-vision services, work tasks or prosthetic support if eye removal is required. Preserving an eye and preserving its detailed vision are different outcomes. [3] [8]
Use disease-specific emotional support
Primary and metastatic ocular melanoma groups provide places to discuss uncertainty with others. CURE OM describes facilitated, non-medical support rather than direct treatment advice; check the current organizer and access arrangements. [7]
Check practical assistance eligibility
Travel, prosthetic and other assistance may reduce barriers, but specific programs can be restricted by location and eligibility. A foundation directory is not a promise of funding or proof that a listed clinician is the best fit. [8]

COMS medium-tumor randomized comparison
Did not demonstrate a survival difference between the randomized approaches through the reported long-term follow-up. [3]
Limits and applicability: Specific tumor-size eligibility and historical treatment context. Similar survival does not establish equal visual function, local complications or suitability for every lesion.
Tebentafusp phase III: three- and five-year follow-up
The three-year report found median survival of 21.6 versus 16.9 months. The 2026 extended report estimated five-year survival at 16% with tebentafusp versus 8% with investigator-selected control therapy. [6] [11]
Limits and applicability: Repeated reports from the same 2021 trial cohort are not independent replications. The selected genotype, metastatic setting, adverse effects, subsequent therapies and sponsor involvement limit broader extrapolation. Exploratory ctDNA and post hoc treatment-beyond-progression associations do not establish an individual prognosis or a causal reason to continue treatment after progression.
FOCUS evidence supporting HEPZATO KIT approval
FDA reported an objective response rate of 36.3% and median response duration of 14 months, supporting a restricted US indication. [5]
Limits and applicability: No concurrent randomized comparator in the approval dataset; response is not proof of cure or a general survival advantage. Serious procedural and marrow risks matter.
Questions researchers still need to answer
- Which systemic or adjuvant approaches can safely reduce metastatic risk after local treatment?
- How can surveillance and biomarkers improve patient-important outcomes while limiting burden and uncertainty?

Questions you may still have
Can the tumor exist without visual symptoms?
Yes. An internal lesion may be found on routine examination before vision changes are noticed. A clear simulation cannot exclude it. [1] [2]
Does a dark eye freckle mean cancer?
No. A choroidal nevus is generally benign, and lesion risk requires specialist examination and imaging rather than visual appearance alone. [2]
Does local tumor control end all follow-up?
No. Eye follow-up and systemic surveillance have different roles and may continue after successful local treatment. Ask for an individualized schedule and contact plan. [3]
Can every patient receive tebentafusp?
No. The cited US indication requires adult unresectable or metastatic uveal melanoma with HLA-A*02:01 positivity. Eligibility and access also depend on clinical and jurisdictional factors. [4]
